How to replace your lost health card

You checked all your pockets and bags and all the nooks and crannies in your apartment to no avail.  Ok, so you lost your health card. Don't worry, replacing it isn't too hard. It can be done in less than an hour (depending on how busy they are). How do I know?  My health card had once disappear too.
All you need to do is to go to the National Health Insurance (中央健康保險局)  building, bring your ARC, $200 and a smile (the smile is optional).  You won't need to bring a photo because they will take a picture of you there (thus the smile ).

I went by myself.  I did have a Taiwanese person write a note that said I lost my health card.  That was all the Chinese I needed.  The workers I talked to had limited English.  It was enough.
  1. When you walk in, you will need to go to the counter to the right.  This is where they will take down your information and take your picture.
  2. They will then hand you a slip of paper that has a number on it. When your number is called, go to the appropriate counter. 
  3. You will need to pay $200 here.  This person will ask what type of job you have and then have you sign a form. They will then give you another slip of paper that contains a number.
  4. Look to the counter position on the far left.  When your number appears here, your card is ready.
  5. You are now a proud owner of a new health card.
